For Incoming Freshmen
* 3 pcs. 1 x 1 ID picture
* High School Identification Card
* 2 pcs. pencil with eraser
* 1 pc. black ballpen
For Transferees/Shiftees/ SecondCoursers
* Transcript of Records
* Honorable Dismissal
* Certificate of Good Moral Character from previous school
* Written Request to Transfer/Return/Shift addressed to the Dean of the desired program
* 3 pcs. 1 x 1 ID picture
* High School Identification Card
* 2 pcs. pencil with eraser
* 1 pc. black ballpen
Please visit the Admissions Office for an initial interview an hour before your preferred day and time slot.
For transferees shiftees or second coursers, you are required to see the Dean or Chairperson for evaluation of records and interview.
