Enrollment Procedures

Procedures for Admission of Incoming Freshmen and Transferees

 Procedures/ Requirements for Entering Freshmen

                1.      See the Admissions Officer and present the following:

·        accomplished Admission Application Form;

·        three (3) pieces 1 x 1 ID picture


2.      Take the entrance exam and secure the favorable result from the Guidance Office


3.      Present the official copy of NCAE/NSAT Results and undergo interview and consultation with the Guidance Counselor on his/her preferred course.


4.      Guidance gives advice and allows the student to proceed with his/her physical exam.


5.      Pay the physical exam fee at the Accounting Office and get Referral Slip for the Physical Examination at the School Clinic.


6.      Go to the Laboratory and X-Ray Department at the hospital for the required test. Go to the school physician for a Physical Examination.


7.      Present the Admission Slip to the School Clinic to get results of the Physical Examination.


8.      Proceed to the Registrar’s Office for submission of the following documents:

·        Admission Slip

·        DepEd Form 138 or High School Report Card

·        Photocopy of the NSO Birth Certificate

·        NCAE/NSAT Results

·        1 long size brown envelope


  9.  Proceed to the Accounting Office for payment.


10.  Proceed to the IMC for school ID.


11.  Proceed to the Discipline Office for submission of the Information/Agreement Form and release of the ID.

 

NO ENTRANCE EXAM FEE!!!

*Applicable to all incoming freshmen and transferees.

 


Procedures/Requirements for Transferees

 

Transferees are those applicants who have earned collegiate or technical level (ladderized program leading to a baccalaureate program) credits from another school of university.

 

The applicant must have passed all subjects found in the curriculum of the course applied from his previous school.  He/she must not incur a ‘No Grade, “nor an “Incomplete” in any course or subject.

 

1.      See the Dean of the College for:

·        a personal interview;

·        evaluation of documents; and

·        accreditation of subjects (depending on the school and program where the equivalent subjects were earned and grades obtained).


2.      See the Admissions Officer and present the following;

·        accomplished Admission Application Form;

·        letter indicating reason for transfer;

·        Transcript of Records;

·        Transfer Credential/Honorable Dismissal

·        Certificate of Good Moral Character signed by the Dean or Authorized Officer of the school last attended.


3.      Take the entrance exam at the Guidance Office and secure favorable results.


4.      Pay the physical exam fee at the Accounting Office and get Referral Slip for the Physical Examination at the School Clinic.


5.      Go to the Laboratory and X-Ray Department at the hospital for the required Physical Examination.


6.      Present the Admission Slip to the School Clinic to get results of the Physical Examination.


7.      Proceed to the Dean’s Office for approved list of academic load.


8.      Proceed to the Registrar’s Office for submission of the following documents:

·        Admission Slip

·        Transcript of Records

·        Honorable Dismissal

·        Photocopy of the NSO Birth Certificate; and

·        1 long size brown envelope


9.      Proceed to the Accounting Office for payment.


10.  Proceed to the IMC for school ID.


11.  Proceed to the Discipline Office for submission of the Information/Agreement Form and release of the ID.
